• Team Check-in Instructions: ONLY 1 PERSON NEEDS TO CHECK IN THEIR TEAM.
   All teams are required to check in on Saturday, October 24^th,
•  at the White Headquarters Tent at Pacesetter Park, located at 8801 Sylvania Metamora Rd., Sylvania, you will need to report at least 1½ hours prior to your first scheduled match.  This will be the only time to check in your team for the tournament. 
• There will be no Friday night registration. 
  You will be required to provide the following to the registration tent on October 24th:
• Two (2) typed copies of your Official Roster, including jersey numbers. (The rosters must be properly validated and cannot exceed the maximum player or coach requirements.)
• One typed guest player rosters, if applicable.  (The rosters must be properly validated and cannot exceed more than three.)
• Current player passes. (The passes must be properly validated, and contain pictures and signatures of all players and coaches.)
• A completed Liability Release Form (www.festivalsoccer.com).  Signatures of all participants’ guardians must be included.
•   Notarized Medical Release Form (www.festivalsoccer.com) for each player, including guest players.
• Travel Permit. (Only needed if team is outside of OYSAN.)
